CHILDCARE POLICIES AND PROCEDURES
1. For the well-being of the other children, please refrain from bringing a sick child into the Facility.
2. Maximum time allowed per day is 8 hours (drop-in)10 am-6 pm.
3. Please label all of your child’s belongings with his/her name.
4. MCAN will provide age-appropriate toys. For safety and hygienic reasons, we ask that children refrain from bringing their own toys.
5. Children will remove shoes while in care. Please feel free to bring a pair of socks for comfort.
6. No food or drink will be permitted in play areas.
7. Disruptive or inappropriate behavior will not be tolerated. In these circumstances, parents will be notified immediately.
8. Children will be released ONLY to the person who is a registered parent and trainee of the program.
9. Medications will not be administered by childcare staff except for life-threatening emergencies.
LIABILITY WAIVER
I, the undersigned, hereby hold harmless, waive, and release Massachusetts Communities Action Network, their childcare workers, volunteers, employees, and organizers from liability as a result of personal injury or property damage occurring while the above child/children are participating in childcare. I understand that childcare services are provided for free as they relate to the training. I have read, understand, and agree to adhere to the childcare policies and procedures of the Massachusetts Communities Action Network, listed above.
PHOTO/VIDEO RELEASE FORM
1. I herein authorize Massachusetts Communities Action Network (MCAN) and any MCAN network affiliates to use, edit, copy, exhibit, distribute and use any images or video footage of me from the training for any lawful purpose.
2. The authorization I provide herein extends to all languages, formats, and media that have been discovered now or will later be discovered.
3. The term of this authorization is indefinite and will be active unless I revoke it in writing.
4. Hereby, I also waive my right to approve and/or inspect a product where I appear or my likeness appears.
5. I also agree and state that I have agreed to these Photo/Video Release conditions without compensation.
6. I consent that the materials will be the sole property of MCAN.

POLÍTICAS Y PROCEDIMIENTOS DE CUIDADO INFANTIL
1. Por el bienestar de los demás niños, abstenerse de traer a un niño enfermo al Instalación.
2. El tiempo máximo permitido por día es de 8 horas (sin cita previa) de 10 am a 6 pm.
3. Por favor etiquete todas las pertenencias de su hijo con su nombre.

9. El personal de cuidado infantil no administra medicamentos, excepto en emergencias que pongan en peligro la vida.
EXENCIÓN DE RESPONSABILIDAD
Yo, el abajo firmante, por la presente eximo de responsabilidad, renunció y liberó a Massachusetts Communities Action Network, sus trabajadores de cuidado infantil,
voluntarios, empleados y organizadores de toda responsabilidad como resultado de lesiones personales o daños a la propiedad ocurridos mientras el niño o los niños mencionados anteriormente participan en el cuidado infantil. . Entiendo que los servicios de cuidado infantil se brindan de forma gratuita en lo que se refiere a la capacitación. He leído, entiendo y acepto cumplir con las políticas y procedimientos de cuidado infantil de Massachusetts Communities Action Network, enumerados anteriormente.
FORMULARIO DE AUTHORIZACION DE FOTOS/VIDEOS
1. Por la presente, autorizo a la Massachusetts Community Action Network ( MCAN) y a cualquier afiliada a MCAN a usar, editar, copias, exhibir, distribuir y usar cualquier imagen o video mio de esta capacitación de MCAN.
